Referral Express, New User Instructions

If you're a plan provider, or other local medical provider, and would like access to Referral Express for you and / or your staff just follow the simple instructions below and your account(s) can often be setup on the same working day as the request.

1. Notify HPSJ of your interest in setting up an account by phoning our Provider Services department at (209) 942-6340. Be sure to state the full names, of every individual you want to have access.
2. We will provide each individual a Health Plan of San Joaquin Confidentiality Statement which must be filled out and faxed back to HPSJ before the account can be setup.
3. Upon receiving the Confidentiality Statement, each user will receive a call or e-mail presenting them with their username and passwords to log into Referral Express.
