Health Plan of San Joaquin (HPSJ) seeks to award the Health Careers Scholarship to graduating high school seniors who plan to pursue a healthcare or medical career at an accredited four-year college or university, demonstrate an interest in community health, intend to practice in the Central Valley and demonstrate financial need.

Applicant Requirements: Must be a graduating senior at a local high school in San Joaquin or Stanislaus County. Must participate in the Health Careers Scholarship Mentorship Program. Note: If you are applying through University of Pacific - Thomas J. Long School of Pharmacy, Physician's Assistant Program, or California State University - Stanislaus School of Nursing, please apply using your school's scholarship application process.
HPSJ Award Amount: $3,000 per student
Deadlines: Final application submission: Friday, May 31, 2024. Letters of Recommendation: Friday, June 7, 2024.

This is a voluntary and confidential scholarship application form to be used by the scholarship committee. All applicants must have been a participant enrolled in a participating school program. In order to fully understand the applicant’s need and to make equitable choices of scholarship recipients, the information should be completed accurately. All responses must be typed.
